The rise of HA (hyaluronic acid) fillers marks a major shift from earlier, riskier procedures like silicone implants or fat grafting, which carried higher risks of infection, deformity, or permanent nerve damage. In contrast, hyaluronic acid—the same substance used in facial fillers—is temporary, low-risk, and reversible.
“I’m conservative when it comes to treatments,” said Pearlman. “If something doesn’t have a strong safety profile, I don’t offer it. Most of my patients feel the same way—they aren’t chasing extremes. They just want a modest boost in girth to feel more confident without major risks.”

A 2025 study by PhalloFILL, one of the leading developers of HA filler techniques and aftercare routines, surveyed 431 men who underwent the procedure.
